Focal dystonia is treated according to the body region it affects: botulinum toxin injections are first-line for most forms, supported by physiotherapy or retraining, and deep brain stimulation is used for disabling cervical and cranial dystonia when injections fail. For cervical dystonia, DBS improves the standard rating scale by 58.8% on average,[1] and for Meige syndrome by about 60% at 12 months.[2]
For task-specific hand dystonia and isolated voice dystonia, the surgical evidence is much smaller, and DBS is only considered in exceptional cases. A specialist evaluation clarifies which group you belong to.
Treatment by region
| Focal dystonia | First-line | Role of DBS |
|---|---|---|
| Cervical dystonia | Botulinum toxin + physiotherapy | Established. Sham-controlled RCT[3]; 58.8% average improvement[1]. See cervical dystonia treatment |
| Blepharospasm with jaw/face dystonia (Meige) | Botulinum toxin | Established. Randomized double-blind trial[2]. See Meige treatment |
| Isolated blepharospasm | Botulinum toxin | Rarely, in severe resistant cases |
| Oromandibular dystonia | Botulinum toxin (technically demanding) | Considered when disabling, as part of cranial dystonia |
| Laryngeal dystonia (spasmodic dysphonia) | Botulinum toxin by an ENT specialist | Investigational for isolated cases |
| Writer’s cramp, musician’s dystonia | Botulinum toxin, retraining | Limited; thalamic surgery in selected cases at specialised centers |
| Foot dystonia | Botulinum toxin, physiotherapy | Depends on cause; consider when part of a wider dystonia |
When does focal dystonia need DBS?
- When it is disabling despite optimal injections
- When it involves muscles that cannot be injected well without weakness
- When it spreads to neighbouring regions and becomes segmental
- When pain is not controlled
DBS for focal dystonia is bilateral for the neck and face, because these muscles receive control from both sides of the brain.
Frequently asked questions
Which focal dystonias respond best to DBS?
Is DBS done on one side or both sides for focal dystonia?
For cervical and cranial dystonia, both sides. One-sided surgery may be considered for dystonia limited to one arm or leg.
